Care Coordinator I Registered Nurse – Remedy Recruitment – Victoria

Our client is a well-established home care provider offering a very high level of bespoke service to their clients. The hand-picked team of carers is carefully screened and selected to ensure best client match and highest quality care.

We are seeking an experienced and highly professional Care Coordinator to join the team. In this clinical leadership position, you will participate in client on-boarding, selection and rostering of a compatible team of carers. As part of a team of Clinical Care Coordinators you will manage your own portfolio of clients, schedule regular visits and client communication. Our ideal candidate is someone who will be excited about being part of an organisation who really listens to their clients, who is compassionate and recognises the importance of authentic engagement and taking the time to build trusted relationships.

This position will suit a Registered Nurse with strong clinical skills, an interest in palliative care, dementia, chronic disease and a commitment to delivering quality personalised care and gold standard customer service.

This is a full-time position Monday – Friday

Care Coordinator Profile:

  • Current RN Division 1 AHPRA Registration
  • Minimum of 3- 4 years community nursing experience
  • Knowledge and experience with Home Care Packages – highly regarded
  • Computer literacy and experienced with software programs
  • A true desire to spend time with and get to know clients and their families
  • Excellent communications skills – written and verbal
  • Must have a high level of attention to detail
  • Well-presented and professional
  • High level of customer service
  • Current National Police Check
